Symbiotic six armed monstrosity, Dr. Octopus
As part of an experiment, four metal arms were grafted to the bones of Otto Octavius. These caused him to go crazy a little. Soon afterwards, an Alien Costume infected him, causing him to go insane. Calling himself Doctor Octopus, he joined Aether's symbiote army.
