Otto Octavius’s father, Brian, was always very cold and unloving. He downplayed Otto’s intelligence and when he reached puberty, his father began beating him. It was then that Otto discovered up until that point, his mother had been taking the brunt of things. One day, Otto fought back and ended up in the hospital. However, his father was arrested, finally freeing him and his mother from his tyranny.

As Otto grew up, he rediscovered his love for science and, after receiving multiple doctorates, began working at Oscorp. Over time, it became clear that he was starting to suffer from a degenerative nerve disease as a result of his years of beatings. Otto then dedicated all of his time and resources to developing advanced prosthetics to help people like him.

However, when Norman Osborn cut his funding, Otto became desperate. He opted to test his experimental mechanical tentacles on himself, unaware that it had been tampered with by the Green Goblin, who anticipated this course of action. The bonding process was difficult for Otto, frying the reasoning parts of his brain. But this wouldn’t become clear right away.

Still with nobility in his heart, Otto tried to use his newfound abilities with the tentacles to become a hero, and was dubbed Doctor Octopus. He even had a few team-ups with Spider-Man, who admired his intelligence. However, when his power cells began depleting, he realized his only course of action was to steal the necessary resources from Oscorp.

When Spider-Man discovered him in the act of stealing, Otto snapped and attacked him. From there, Doctor Octopus began to develop an unhealthy obsession with Spider-Man for “being in league with Osborn and trying to destroy his life”. After multiple confrontations between them, Doctor Octopus was arrested and placed in S.H.I.E.L.D. custody.

While incarcerated, Doctor Octopus began formulating a plan. The Raft continued to fill up with Spider-Man’s enemies, and when everything fell into place, Otto organized a breakout. He freed and recruited Sandman, the Rhino, Electro, Mysterio, and the Scorpion with the promise that if they worked together to kill Spider-Man, he would help each of them with their own problems.

Forming the Sinister Six, they instantly became the deadliest threat Spider-Man had ever gone up against. Unfortunately for the Six, during their first few confrontations, Spider-Man was bonded to the Venom symbiote, which gave him an edge over them. Over the years, the members of the Six would be arrested and broken out a dozen times over, always unable to fulfill their final goal.

Then the day came when they were approached by the psychotic internet sensation Screwball, who put forward a plan to put Spider-Man through a gauntlet that would be streamed live on the internet, climaxing in his death. Doctor Octopus was hesitant to trust Screwball, but ultimately agreed.

He was to be the last foe for Spider-Man to go up against, unaware that he was destined to finally kill him by the Prophecy of Infinity. But things were different this time around, as the Avengers interjected themselves into the battle. Doctor Octopus then witnessed Spider-Man jumping in front of a bullet to save Captain America’s life, and for the first time saw Spider-Man for the young, selfless hero he truly was. Unable to deal the killing blow, Doctor Octopus surrendered himself and was once more imprisoned on the Raft.

Believing he would spend the remainder of his days imprisoned, Otto was shocked when he learned he had been selected for the Thunderbolt Initiative. After a conversation with Spider-Man, he learned that he recommended him due to believing Otto still had the potential to do good inside him. He was inspired by Spider-Man's confidence in him, and vowed to prove himself as a worthy hero by working with the Thunderbolts.
